Members Responsibilities: Members are responsible for ensuring the accuracy, currency and completeness of all dependant information. Changes that impact eligibility, that are not notified and which result in the processing of a benefit, will be treated as a breach of the Conditions of Use.
PMKeyS Information: The Dependant/Beneficiary area of PMKeyS is the source for dependant information used to verify dependant eligibility. The ADF Member is responsible for ensuring their dependants details are maintained in PMKeyS. Dependant Details can be added and updated within PMKeyS using the AD160 ADF Dependant Details form.
Change my Address or Contact Information: Once the Dependant / Beneficiary section of PMKeyS has been updated. Please notify the ADF Family Health Team at ADF.DependantHealth@defence.gov.au or contact us on 02 6266 3547.
Add a Dependant: Ensure that the dependant is in the Dependant /Beneficiary area of PMKeyS. New dependants can be added to PMKeyS using the AD160 ADF Dependant Details form.
Additional dependants can be added to the trial by completing an AD858-1 ADF Family Health Trial form and forwarding to the ADF Family Health Team. A new ADF Family Health card will be issued within 14 days receipt of the form or confirmation that the dependant details are correct.
Cancel a Dependant: ADF Members can cancel dependants at any time by notifying the ADF Dependant Health Team at ADF.DependantHealth@defence.gov.au or contact us on 02 6266 3547.
Change a Name: The first step is to update the dependant's name in the Dependant/Beneficiary area of PMKeyS by using the AD160 ADF Dependant Detail form. The ADF Family Health Team should then be notified at ADF.DependantHealth@defence.gov.au or contact us on 02 6266 3547.
Navy Health Ltd - Navy Health will provide claims processing, benefit payment, customer service and card production. To support their customer service, Navy Health may seek additional information. This additional information is not shared with Defence. You can change your information with Navy Health through the Online Members Support Portal at: https://adffh.navyhealth.com.au/ or call on 1300 561 454. Address changes are to be forwarded to the ADF Family Health Team.
